Topics discussed with the liaison nurse

The date of your return home

During medical visits, an end date for the stay is announced. The liaison nurse will schedule a meeting in the days following the visit to ensure that everything is ready for your return home.

Transport

We will discuss with you the most suitable transport for your return home, depending on your state of health and your needs.

Home help

If necessary, we will determine whether you will need assistance with certain daily tasks, such as washing, preparing meals or cleaning.

Home healthcare

If you require medical care after your discharge from the clinic, we will arrange for a nurse to visit you at home.

Make your environment safe

The therapists will have worked with you to assess the technical aids and auxiliary resources necessary for your return home, and the liaison nurse will ensure that the prescription is drawn up accordingly.

Follow-up after rehabilitation

Our therapists will assess whether post-rehabilitation follow-up is necessary and can recommend professionals in your area or those offering home care.

Prescriptions & medicines

The doctor will go over the prescriptions you will need to follow after your discharge. The medical team will give you a 48-hour supply of medication and dressing materials, if necessary, the day before or the day of your discharge.
