Hospital at home – Hospitalisation à domicile (HàD) relieves pressure on hospitals and empowers patients

Since November 2023, the Réseau de l’Arc’s Hospital-at-Home service has complemented the VIVA model, bringing medical care directly to patients’ homes.

Swiss Medical Network, in collaboration with VIVA – Réseau de l’Arc, has introduced an innovative Hospital-at-Home (HàD) programme in the Jura region. The aim is to provide patients with acute or chronic conditions such as heart failure, COPD or unstable diabetes with high-quality medical care at home that is equivalent to hospital treatment.

An interdisciplinary team of specialist nurses, assistant doctors, therapists, nutritionists and pharmacists looks after patients under the supervision of a specialist from the Hôpital de Moutier or Saint-Imier. The team works closely with GPs, health coordinators and outpatient care services to ensure continuity, safety and successful treatment.

The Hospital-at-Home programme relieves pressure on hospitals by keeping beds free for acute cases, while also improving patients' quality of life. Patients can recover in familiar surroundings, resume their daily activities and actively participate in their treatment.

The results of the programme are encouraging. The service is to be gradually expanded, and new technologies such as remote monitoring and data analysis are expected to open up additional opportunities. Hospital-at-Home is thus a central component of VIVA's integrated care.
