Arthroplastie totale du genou assistée par robot versus arthroplastie totale du genou conventionnelle

M. Terzago, G. Rossanaly, K. Seghrouchni and A. Lunebourg.

At the 86th Annual Congress of the Swiss Society of Orthopaedics in Basel, a team from the Clinique de Valère presented a comparative study evaluating early clinical and surgical outcomes following robot-assisted total knee arthroplasty compared with conventional arthroplasty. This study focused on post-operative flexion contracture, range of motion, operating time and analgesic consumption.

This single-centre retrospective study involved 60 patients treated between October 2024 and December 2025, of whom 30 underwent conventional surgery and 30 underwent robot-assisted surgery using the ROSA® Knee system. Both groups received the same Persona® MC implant and were managed according to a standardised protocol for accelerated post-operative recovery.

Robot-assisted arthroplasty resulted in a significantly better correction of flexion contracture at six weeks, with a median value of 0° compared with 7.5° in the conventional group. At three months, patients in the robotic group also achieved greater knee flexion - 130° compared with 120° - although the difference in residual flexion contracture was no longer statistically significant. The duration of surgery was longer with robotic assistance, averaging 98.1 minutes compared with 79.8 minutes.

These results suggest that ROSA®-assisted surgery may improve early soft-tissue balance and accelerate initial functional recovery. However, the retrospective design of the study, the limited sample size, the short follow-up period and the potential effect of the learning curve highlight the need for larger-scale prospective studies to confirm these findings.
